(1) The above offers are good only for "self pay" patients and those with health insurance that does not cover our services. Patients with health insurance that covers our services are bound by the terms of their health insurance contract.
(2) Offer is good for "in-office" services only during normal office hours
(3) The patient or any other person responsible for payment has a right to refuse to pay, cancel payment or be reimbursed for payment for any other service, examination or treatment which is performed as a result of and withing 72 hours of responding to the advertisement for a free or discounted service, examination or treatment.
(4) Initial consultation and examination fee (99202) is regularly $175. Follow-up visit (99211) are regularly $50
